Chronic Care Management Coach

The Chronic Care Management (CCM) Coach at Carda Health plays a critical role in performing chronic care management services under the supervision of the nurse practitioner for patients participating in Phase 3 Pulmonary Rehab. The role involves patient engagement, medical evaluation, and supporting a high standard of care through telehealth.

Location: Remote

Salary: $19.00 - $24.00/hr

Responsibilities

• Ensure the privacy and accuracy of patient information in compliance with HIPAA regulations.
• Maintain medical documentation accurately and timely, ensuring proper organization and confidentiality.
• Patient care in the form of bi-weekly video visits, and communicating with patients through phone calls, text, and email.
• Provide coaching on patient health goals including accountability, motivation, and encouragement.
• Monitor vital signs during sessions and handle live emergencies, as needed.
• Coordinate care for patients; including making appointments and helping with medication clarifications/adjustments from doctor’s offices.
• 1:1 education on diseases, therapies, medications, and resources available to patients.
• Provide general administrative support and assist with other tasks to maintain smooth operations in the medical office.
• Work directly with Carda’s founders and clinical team to innovate and improve digital care delivery model.

Requirements

• Bachelor's degree in Clinical Exercise Physiology / Exercise Science.
• Minimum of two years of experience in a medical office setting, preferably in cardiac and pulmonary rehab.
• Proficiency with medical records systems and software.
• Excellent attention to detail and accuracy in data entry and record management.
• Knowledge of medical terminology and understanding of medical record documentation.
• Strong organizational, problem-solving, and communication skills.
• Familiarity with HIPAA regulations and commitment to patient privacy and confidentiality.
