Remote Care Coordinator - CMA/RMA in MST or PST

Chronic Care Staffing is seeking a compassionate and detail-oriented Remote Care Coordinator - CMA/RMA to support patients managing chronic conditions through proactive outreach, education, and care coordination. This fully remote role involves serving as a trusted point of contact for patients, helping them stay engaged in their care plans while supporting providers through accurate documentation and communication.

Location: Remote (MST or PST Time Zones Only)

Salary: $18–$21/hour

Responsibilities

• Conduct structured monthly outreach calls to monitor patient progress and engagement.
• Verify medication adherence and assist with health risk assessments.
• Support Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) initiatives.
• Document patient interactions, care updates, and clinical information accurately within EHR systems.
• Coordinate communication between patients, families, providers, and internal care teams.
• Identify barriers to care and escalate patient concerns when necessary.
• Educate patients on care plans, treatment goals, and available support resources.
• Maintain HIPAA compliance and CMS documentation standards at all times.

Requirements

• Residency within the PST or MST time zone.
• Active, unencumbered CMA or RMA certification.
• Current Basic Life Support (BLS) certification.
• Recent experience in a clinical, care coordination, or chronic care management setting.
• Strong proficiency with EHR systems and Google Suite.
• Dedicated, HIPAA-compliant home office with reliable high-speed internet.
• Excellent communication, organization, and patient engagement skills.
