LPN Care Navigator (Alabama)

The LPN Care Navigator is a remote healthcare role focused on managing patient panels, providing health coaching, and coordinating care for individuals with chronic conditions. The position requires a compassionate, communicative, and tech-savvy licensed practical nurse based in the US.

Location: Remote (Alabama, US)

Salary: $15.75–$18.00 per hour

Responsibilities

• Manage an assigned panel of 400-450 patients and conduct monthly outreach.
• Conduct 50-60 daily outbound calls to patients for care support and health coaching.
• Maintain call quality standards through professional communication.
• Collaborate with clinical teams, providers, and caregivers to identify and address clinical and social needs.
• Support care coordination goals by staying on schedule with outreach targets.
• Close gaps in patient care through various services like Chronic Care Management, Behavioral Health Integration, and Remote Physiological Monitoring.
• Advocate for patient needs by listening, investigating, and communicating solutions.
• Document all patient interactions in real time using the care coordination platform.
• Maintain HIPAA compliance in all communications and documentation.

Requirements

• Active Licensed Practical Nurse or Licensed Vocational Nurse license with multi-state compact license.
• Willing to obtain company-paid state-specific license if needed.
• Must reside in the U.S.
• Minimum 2 years’ clinical experience.
• Experience working with EMR systems.
• Familiarity with chronic condition management and individualized care planning.
• Intermediate to advanced computer skills.
• Telephonic and virtual communication skills.

Additional Information

• This is a fully remote position with a Monday–Friday schedule, no weekends or late nights.
• Candidates must have high-speed internet (minimum 50 Mbps download / 5 Mbps upload) and a private workspace for HIPAA compliance.
• Candidates must reside in the U.S. and have a suitable home office setup.
