Salary: $20–$24/hr depending on experience
Responsibilities
- Provide advanced coordination across multiple patient care pathways using AI tech-enabled workflows
- Establish relationships with and serve as primary point of contact for patients and their family or caretaker.
- Assist patients with digital forms and surveys, utilizing AI tools to streamline the process.
- Facilitate patient engagement through proactive outreach and digital communication tools.
- Clearly communicate the purposes and services available to patients, family members, and caregivers.
- Coordinate transportation into or out of a care setting.
- Maintain contact with client or hospital/clinic leadership for initiatives.
- Aid in customer service, patient experience, and relay feedback or issues.
- Investigate and resolve patient/provider inquiries and concerns.
- Ensure compliance with HIPAA and protect patient information.
- Develop expertise in insurance benefits and exclusions.
- Document all encounters and complete reporting.
- Enter detailed information into proprietary software.
- Disclose training, experience, and credentials.
- Analyze patient data to identify care gaps.
- Collaborate with AI tools for patient interactions.
- Meet KPIs including service levels, call volumes, adherence, and quality.
- Attend team meetings.
Requirements
- High school diploma or GED
- Experience in healthcare, home health, managed care, behavioral health, population health, or care coordination
- Experience in high-accountability remote operations environments
- Ability to maintain high productivity autonomously
- Experience working in a call center environment
