Case Coordinator Family Supports

The Case Coordinator provides comprehensive assessments and develops care plans for clients with developmental disabilities, supporting client self-management and coordinating community and residential services.
Salary: $27–$28
Responsibilities
- Interpret and complete standardized assessment tools such as the interRAI CHA, supports Intensity Scale, Application for Developmental Services, or other assessment tools as appropriate.
- Complete client assessments using conversation based methodology.
- Develop and conduct a comprehensive therapeutic rapport.
- Identify and prioritize environmental risk for client’s health and safety.
- Develop a comprehensive care plan interpreting the Clinical Assessment Protocol and care planning.
- Facilitate access to client identified services ensuring navigation to appropriate resources.
- Make referrals to community and health resources as required.
- Consult with partners in care service plans with client consent.
- Monitor client progress and adjust care plans as required.
- Review client care plan expenditures and support clients to manage within the client care budget.
- Access and review assessments.
- Collect and report relevant data to Manager.
- Adhere to privacy policies and standards.
- Represent providers and community agencies professionally.
- Mediate and problem solve with multiple parties.
- Coordinate and provide Family Home support.
- Ensure referrals and support in emergency situations.
- Participate in screening and matching of new Family Home Providers.
- Promote VON’s goals and values.
- Ensure workplace safety and abide by policies.
- Collaborate with staff and external partners.
- Engage with program coordinators and managers.
Requirements
- University Degree or College Diploma in Health or Social Services field.
- Minimum of 3 years related experience in a home and community care organization or social services program including experience in case management.
- Experience in completing validated assessment tools.
- Broad knowledge of community resources.
- Knowledge and skills in assessment and maintaining proficiency.
- Experience working with volunteers and seniors is an asset.
